RE: Form 9. Conditions for Sentence to Probation.
Attached are proposed amendments to Form 9, otherwise known as Appendix "A". The amendments were recommended by the Field Services Division of the Department of Correction and Rehabilitation.
In preparing the proposal for the Committee, staff made only form and style changes to the recommendations made by Field Services. The letter from Field Services indicates that it is seeking a uniform form for use statewide that incorporates optional additional conditions for sex offenders.
