The mental health commitment forms are being offered in Rich Text Format (RTF) and Adobe Portable Document Format (PDF) versions.

The RTF format is intended to allow the document to be opened in a word processor, completed within the word processor then printed for filing.

The PDF version is intended to allow the document to be printed and filled “by hand”, then filed. To retrieve the document, click the appropriate document name on the links page. The document should be returned to your web browser and opened in the appropriate application, such as Word, WordPerfect or Adobe Acrobat. Once in the application, the document should be saved to your local data drive and directory.

If, when clicked, the document does not open in the appropriate application, you should Right click on the document, then click “Save target as…". In the dialog box that appears, specify where you wish to save the document and the name of the document. Then, you can open the appropriate application (Word, WordPefect, or Acrobat) and open the document from within the application.
