Terminate (End) Guardianship of Minor – 18 Years Old, Adopted, Married or Deceased

The Guardian/Co-Guardians may use these forms if:

  1. A North Dakota state district court or North Dakota state juvenile court appointed the guardian/co-guardians for the child; AND
  2. The guardian/co-guardians want to ask the North Dakota state court to terminate or end the guardianship for one of the following reasons:
    • The child turned 18 years old;
    • The child was adopted;
    • The child was married (and is under 18 years old); OR
    • The child died.
