Terminate (End) Guardianship of Minor – Parents Withdrew Consent, or Reasons No Longer Exist

These forms may be used by:

  • The child’s parent (if their parental rights haven’t been terminated by court order);
  • The guardian (or co-guardians);
  • The child, if the child is 14 years old or older; OR
  • An interested person

To ask a North Dakota Juvenile Court to terminate (end) the guardianship of the minor child and discharge the guardian (or co-guardians) because:

  • The parents originally consented to the guardianship, but now withdraw their consent; OR
  • The basis (reason) for the guardianship no longer exists.
