Request Certificate of Professional License

The Certificate of Professional License (SFN 13610) is an official document used to verify professional credentials with the North Dakota Secretary of State. This certificate is required at the time of some professional organization formation and, pursuant to changes effective in 2026, must be filed on an annual basis.

Submission Requirements

Prior to submission, Sections 1–4 of SFN 13610 must be completed in full. The form may be submitted via email to [email protected] or by mail accompanied by the required fee. Please provide a valid email address to ensure the certified form can be returned electronically.

Fee Schedule

A fee of $20.00 per professional organization is required. Payment must be submitted by check made payable to the State Board of Law Examiners and mailed to the address below. Cash and credit card payments are not accepted.
